Release Of Liability, Assumption Of Risk, And Media Release

This agreement is between Bravo Dance School, its instructors and authorized representatives (“Bravo”), and the undersigned participant.

I agree as follows:

1. I am voluntarily participating in classes, activities, socials, and rehearsals offered by Bravo. I understand that dance and fitness activities involve physical exertion and inherent risks, including serious injury, property damage, and death.
2. I acknowledge that it is my responsibility to consult a physician before participating. I represent that I am physically able to participate and have no condition that would prevent my safe involvement.
3. I voluntarily assume all risks, known and unknown, associated with my participation and accept full responsibility for any injury, death, property damage, or other loss that may result.
4. In consideration of being permitted to participate, I knowingly and voluntarily release, waive, discharge, and covenant not to sue Bravo Dance School, its instructors, and authorized representatives from any and all claims, liabilities, demands, damages, or causes of action arising from or related to my participation, including claims resulting in injury, death, property damage, or other loss.
5. This Release is binding upon me and my heirs, executors, administrators, legal representatives, and assigns.
6. I grant Bravo permission to photograph, record, and use my likeness, image, voice, and appearance in any media for promotional or other lawful purposes without compensation. I waive any right to inspect or approve such materials and release Bravo from any claims arising from their use.
7. I have carefully read and fully understand this Release. I understand that I am giving up substantial legal rights, including the right to sue, and I sign this agreement freely and voluntarily. This Release remains in effect for as long as I participate in any Bravo class, activity, social, or rehearsal.
